About

Connect Bassetlaw seeks to support people to address issues such as unstable and inappropriate accommodation, loneliness and isolation and financial uncertainty. The support on offer is varied and includes brief interventions, time-limited support and intermittent long-term, low-level care.

The team can provide support remotely, face-to-face or in group settings.

The principle aim of the service is to prevent, reduce and delay the need for statutory Adult Social Care support, and to enable people to be as independent as possible building upon their own strengths and opportunities, and those of their family and community.

Eligibility and referrals

Connect Bassetlaw supports the needs of people in the following groups:

• People aged 17+
• Older people and people with long-term conditions
• People with low to moderate learning disabilities
• Autistic people and those with neuro-diverse conditions
• People with an acquired brain injury
• People presenting with multiple concurrent low-level needs.

The service does not support people who are under 17 years of age and is unable to carry out personal care or health care tasks.
